Care Management Solutions
Care Management Solutions strives to ensure peace of mind for the client and their families and to bring balance to family life by offering concierge nursing, care management and personal independent caregiving services. At CMS, we offer a holistic nursing approach, i.e. emotional, physical, spiritual, financial and social supports, offerings and resources. A CMS care manager will assess and ensure that all of these pieces are balanced between the client, the family, homemaker/companion, medical providers, insurance companies and all involved in the care of each client.

Concierge Case Management
We provide family communication, assistance with long-term care insurance policies, coordinate additional caregiver training, caregiver coaching and counseling, end-of-life transitions, assistance with resources for financial care planning, and home transitions.

Independent Contractor Monitoring Program
We provide options for caregiver management;

Tier 1: We provide an independent caregiver contractor locator service dedicated to sourcing strong caregiver talent.

Tier 2: Our independent caregiver startup program comprehensively locates, vets, and orients independent contractors. The program offers meet-&-greets, care plan development, and first day training.

Tier 3: Our monitoring service begins after the first date of caregiver service and includes weekly operations to include devising a client-driven schedule; tracking hours for invoicing for client direct pay.

Concierge Nursing
As a part of our concierge nursing service, we provide home visits which include reviewing health indicators/assessments, psychosocial status checks, and medication management. We also aid with on-call emergency assistance if needed, and illness/injury prevention/progression monitoring.

At Care Management Solutions, we believe health care is not just a system—it's a human experience. Too often, however, the burden of navigating care falls hardest on those with the fewest resources: the elderly, patients recovering from surgery, those with low health literacy, language barriers, or limited family support. Therefore, we aim to address the systemic gaps that affect Connecticut residents and continue to advocate for better care models, clearer communication, and systems that treat clients with the dignity they deserve.

Whether it's a lack of access to technology, confusion over discharge instructions, or the emotional overwhelm that comes with recovering alone, we firsthand see how gaps in the health care system can become barriers to healing. That's where advocacy comes in.

Whether someone is coming home from the hospital or rehabilitation, navigating follow-up care, facing illness or injury recovery at home, or needing assistance with monitoring illness progression, we believe that health care advocacy and equity must guide how support is delivered: in person, in plain language, and attuned to each patient emotional and physical readiness.
