Care Management Solutions

Care Management Solutions strives to ensure peace of mind for the client and their families and to bring balance to family life by offering concierge nursing, care management and personal independent caregiving services. At CMS, we offer a holistic nursing approach, i.e. emotional, physical, spiritual, financial and social supports, offerings and resources. A CMS care manager will assess and ensure that all of these pieces are balanced between the client, the family, homemaker/companion, medical providers, insurance companies and all involved in the care of each client.

Concierge Case Management

We provide family communication, assistance with long-term care insurance policies, coordinate additional caregiver training, caregiver coaching and counseling, end-of-life transitions, assistance with resources for financial care planning, and home transitions.

Independent Contractor Monitoring Program

We provide options for caregiver management;

Tier 1: We provide an independent caregiver contractor locator service dedicated to sourcing strong caregiver talent.
Tier 2: Our independent caregiver startup program comprehensively locates, vets, and orients independent contractors. The program offers meet-&-greets, care plan development, and first day training.
Tier 3: Our monitoring service begins after the first date of caregiver service and includes weekly operations to include devising a client-driven schedule; tracking hours for invoicing for client direct pay.

Concierge Nursing

As a part of our concierge nursing service, we provide home visits which include reviewing health indicators/assessments, psychosocial status checks, and medication management. We also aid with on-call emergency assistance if needed, and illness/injury prevention/progression monitoring.

At Care Management Solutions, we believe health care is not just a system—it's a human experience. Too often, however, the burden of navigating care falls hardest on those with the fewest resources: the elderly, patients recovering from surgery, those with low health literacy, language barriers, or limited family support. Therefore, we aim to address the systemic gaps that affect Connecticut residents and continue to advocate for better care models, clearer communication, and systems that treat clients with the dignity they deserve.

Whether it's a lack of access to technology, confusion over discharge instructions, or the emotional overwhelm that comes with recovering alone, we firsthand see how gaps in the health care system can become barriers to healing. That's where advocacy comes in.

Health care equity means recognizing that not all patients start from the same place, and making sure no one is left behind.

Our approach to health care is built on relational trust, emotional clarity, and cultural humility. We take the time to listen, to clarify, and to show up, especially during moments of transition, when patients are most vulnerable. Whether someone is coming home from the hospital or rehabilitation, navigating follow-up care, facing illness or injury recovery at home, or needing assistance with monitoring illness progression, we believe that health care advocacy and equity must guide how support is delivered: in person, in plain language, and attuned to each patient emotional and physical readiness.

Owner, Care Manager

Vickie Adorno, NHA, TRS

Vickie’s career began as a Certified Therapeutic Recreation Specialist, specializing in adaptive equipment and brain injury, acquired through a Bachelor of Science program through the School of Nursing at the University of Southern Maine. The project Vickie chose for her required internship was to mainstream 17 geriatric patients from the Augusta Mental Health Institute into a small town community within six months. The program was a great success in which all 17 patients, though trying at times, acclimated to their new surroundings and became slowly accepted by the residents of the community. From this project, Vickie realized that the true meaning of “home” is the comfort and security that someone feels in their “home” environment.

Vickie’s career continued at the Center for Brain Injury in Kennebunkport, Maine for the next six years. Here, she learned to put all the components of her training into action. In this environment, Vickie worked with the social/emotional, socio-economic, physical, and mental/cognitive pieces of her training. It is this part of Vickie’s career where she truly acknowledged the compassion that she felt in helping people acquire the lifestyle that they deserved.

In 1996, Vickie moved from Maine to Connecticut and worked on the psychiatric floor of Middlesex Hospital, in Middletown. Vickie embraced this journey as she developed empathy for the many unfortunate situations that people faced in being homeless, living in unacceptable situations or being moved from one facility to another. There were several elderly people whom Vickie became close to and realized that she wanted to go back to assisting aging people to grow “old” (a relative term!) with dignity and respect.

In 2001, Vickie completed the 2 year, postgraduate, Nursing Home Administrator Program. Six months later, Vickie passed the board exams and acquired her license to be a nursing home administrator in Connecticut. Vickie’s goal as a nursing home administrator was to make each nursing home a place where her own parents, one day, could live with the respect, dignity, independence and comfort that they deserved. Vickie realized that nursing homes are good for many, but others want to return/stay at home. To balance these outcomes, Vickie developed in 2010 a concept for care management, nursing care and homemaker/companion care to come together. This concept later became Care Management Solutions.

Registered Nurse & Care Manager

Jessie Hatfield, BSN, RN

Jessie Hatfield, BSN, RN, began her nursing career as a CNA and shortly thereafter she worked her way to becoming a Licensed Practical Nurse before advancing her education with an Associate Degree in Nursing and later a Bachelor of Science in Nursing. Her career spans long-term and short-term care, clinical research, medical-surgical and oncology units, the emergency department, home health, and a rural island medical clinic. This diverse background gave her a comprehensive view of patient needs across the continuum of care and prepared her to excel in one of her strongest areas—case management. Jessie is skilled at coordinating care plans, advocating for patients, and connecting individuals with the resources they need to improve outcomes and quality of life.

Her mission as a nurse is to deliver compassionate, evidence-based care while empowering patients and families to navigate the health care system with confidence and dignity.

When not working, Jessie enjoys family and personal life, as well as spending time with her grown children, family, friends, and her beloved golden retriever, Fender. Her adventurous and family-centered lifestyle reflects the same values she brings to her nursing career—resilience, adaptability, and genuine care for others.

On Call Phone Line Supervisor

Alexandra Cerquera, CNA

Alexandra Cerquera has over ten years of experience as a Certified Nursing Assistant (CNA) and is passionate about helping others live with dignity. She has completed certifications in Anger Management and Conflict Resolution, Domestic Violence, and Supporting Individuals with Intellectual Disabilities and Mental Illness. She is currently studying Foundations of Nursing Practice to continue expanding her skills.

Outside of work, Alexandra enjoys spending time with her toy poodle, exercising, swimming, and practicing aquatic therapy. Bilingual in English and Spanish, she also loves singing and dancing as a way to recharge. With both professional and personal experience in disability, Alexandra is dedicated to ensuring every individual feels respected, included, and valued.

Certified Nursing Assistant/Assistant Care Manager

McKayla Olechnicki, CNA

McKayla Olechnicki is a Certified Nursing Assistant (CNA) and currently studying at the Arizona College of Nursing in East Hartford, Connecticut, where she's working toward her Bachelor of Science in Nursing. She has about a year and a half left in her program, and is excited to keep growing as she prepares for a future career in nursing.

Caring for others has always been important to McKayla, and working as a CNA has only strengthened that passion. She loves being able to make a difference in someone’s day, whether through compassion, attentiveness, or simply showing kindness.

Outside of school and work, McKayla enjoys spending time outdoors and being creative. Cooking is one of her favorite hobbies, she loves trying out new recipes and sharing meals with loved ones. She also enjoys kayaking, relaxing at the beach, and traveling whenever she can.

McKayla would describe herself as kind, respectful, and passionate, with a strong drive to keep learning and improving. Her ultimate goal is to build a meaningful career in nursing where she can combine skill with compassion to make a positive impact in people’s lives.

Health Care Programs & Operations Manager

Hannah Schilling, BA

Hannah Schilling recently graduated from Bucknell University with a double major in Sociology and Philosophy and is a member of Phi Sigma Tau, the International Philosophy Honor Society. During her time at Bucknell, she was also a dedicated member of both the Bucknell Dance Company and the Bison Girls Dance Team, combining her passion for the arts with her academic pursuits. Hannah further broadened her perspective through a semester abroad in Copenhagen, where she deepened her interest in social systems, cultural perspectives, and equity in healthcare.

At Care Management Solutions, Hannah brings together her academic foundation with real-world experience to assist with care coordination, client advocacy, and program development. Guided by a passion for mental health, trauma-informed practice, and social justice, she combines a systems-level perspective with personal commitment to ensuring clients never feel overlooked or unheard. Hannah is particularly interested in how healthcare systems can better support individuals and families through transitions of care, and she is passionate about advancing healthcare equity and accessibility. These experiences have inspired her long-term goal of pursuing a master’s degree in Social Work or Public Policy, with a focus on health care advocacy and systemic change.

Outside of her professional and academic roles, Hannah enjoys dancing, writing, and spending time with her Mini Goldendoodle, Millie. She is committed to blending creativity, compassion, and critical thinking in her work, and dedicated to shaping systems rooted in dignity, inclusion, and justice that truly serve people in their everyday lives.
